In APA Style, the cover sheet is called a title page. Students should follow the guidelines of their instructor when determining the proper format for their title page. If not instructed otherwise, the following elements are to be included:

  • title of the paper;
  • name of each author of the paper;
  • affiliation for each author (typically the university attended);
  • course number and name for which the paper is being submitted;
  • instructor name;
  • assignment due date; and
  • page number (top right).
