In APA Style, the cover sheet is called a title page. Students should follow the guidelines of their instructor when determining the proper format for their title page. If not instructed otherwise, the following elements are to be included:
- title of the paper;
- name of each author of the paper;
- affiliation for each author (typically the university attended);
- course number and name for which the paper is being submitted;
- instructor name;
- assignment due date; and
- page number (top right).
